Medical Spanish: ED Psych Pre-screening Questions

Hi my name is Howie. I am going to be your nurse for today.

Before we start, I’m going to ask you some pre-screening questions just to make sure we are “all on the same page.”

But first we are going to attach you to our vital signs monitor so we can watch your heart rhythm & rate, your oxygen level, and blood pressure. It will just take a minute.

Now I am going to ask you the same questions I ask of everybody, ok?

Let’s check if your wristband is correct. We want to make sure that you are the right person so that you get the correct medications.

INTRO

  1. What is your full name? Last name?
  2. When is your birthdate?
  3. What year is it now?
  4. What is the name of this hospital?

Perfect. Now I will ask some basic general questions.

TRAVEL & EXPOSURE

  1. Do you know, or think you know, if you may have been exposed to someone who has had COVID or the flu within the past two weeks?
  2. Have you traveled outside the country within the past month?

LIVING SITUATION

  1. Are you coming from home, a skilled or assisted nursing facility, or a board & care?
  2. Do you feel safe at home?
  3. Who else lives with you?
  4. How did you arrive here?

CODE STATUS

  1. This [question] is just a formality, but if we have to resuscitate you, will you allow us to do that? Do you also want us to do chest compressions, or is everything else ok?
  2. If not, do you want us to allow natural death (i.e., DNR = do not resuscitate)?

BLOOD ADMINISTRATION

  1. You do not need to get blood right now. But if we ever need to give you blood, is that ok? If you have objections, may I ask what it is [Jehovah’s Witness, etc.] so that I can put it in your medical chart?

ALLERGIES?

  1. Do you have any medication or food allergies?
  2. If so, what are they and what happens to you when you are exposed to them?

PSYCH

  1. Are you feeling suicidal right now?
  2. If so, do you have a plan?
  3. Do you hear or see anything that may or may not be there?
  4. Are voices from an unknown source telling you to do something?
  5. Do you want to hurt yourself or anyone else right now?

PRE-ASSESSMENT

  1. So, tell me in your own words, what brings you in [the ER] today?
  2. What’s bothering you?
